Client Intake Form
Initial intake form - required to be completed by all new Clients prior to first appointment.
We welcome new Clients and ask that they complete and return our Client Intake Form at least three days before their first consultation. This gives your clinician time to review your information thoroughly and ensures your time together is productive and beneficial.
